Use the form below to register your team for this year's Silver Puck Challenge. There are a few spots available. Bracket* A Bracket B+ Bracket B Bracket Over 40 Bracket Womens Bracket Team Name* Team Manager* Phone* Email Address* MUST READ: There are two (2) options for playing in this year's tournament. The first option is just the tournament with no shirts included. Shirts can be purchased seperately. The other option is select the tournament and shirts. If you select this option you can submit two (2) company logos to be placed on the shirt and website. All members on the team will receive a tournament shirt. Tournament Entry Fees* $575.00 Tourney Only $675.00 Tourney and Shirts If you selected the option for your team receiving shirts please indicate below how many of each shirt size you want. There is a poll to select the top two shirt designs desired, so vote if you haven't already. Shirts will be set a side for you team and can be traded in for the desired design that the player wants.
